Once you submit a referral, we’ll reach out for a brief, complimentary conversation with no obligation. This gives us both an opportunity to ask questions, explore your goals, and ensure our service is the right fit for you.
This will involve us spending some time getting to know you, your situation, and the goals (summits) you want to reach. Using practical assessment tools, we map out a structured therapy plan, connect with key people in your support network for a coordinated approach, and determine if incorporating our Therapy Assistant will be beneficial for you.
Common goals we can work on include:
🏡 Independence & Life Skills – Such as: Personal care, home organisation, financial management, and transport...
🔨 Work & Vocational Engagement – Such as: Workplace readiness, executive function tools, and study routines...
🌍 Social & Community Connection – Such as: Navigating social interactions, building relationships, and group involvement...
🌻 Emotional Regulation & Wellbeing – Such as: Developing self-regulation strategies and healthy daily routines...
This active phase focuses on building practical skills, establishing routines, and applying strategies directly to your everyday life. When beneficial, intervention is delivered through a collaborative model: our Therapy Assistant handles regular, hands-on sessions while our Occupational Therapist provides clinical direction, targeted input, and ongoing oversight.
Depending on your goals, ongoing intervention may include a mixture of:
💻 Targeted Virtual Sessions – Direct work with our Occupational Therapist or Assistant using tools like Motivational Interviewing and Executive Function Coaching to tackle barriers and build practical skills.
🧩 Custom Systems & Resource Development – Such as: Personalised programs, visual planners, and step-by-step guides for you and your support network to use anytime...
🤝 Carer & Support Network Training – Equipping family, informal carers, and support staff to implement strategies consistently across all environments.
